Objective The scope of related research on ethical conflicts in the interprofessional collaboration in nursing was reviewed,so as to summarize the obstacles and coping strategies that lead to ethical conflicts in interprofessional collaboration and provide references for promoting nurse engagement in interprofessional collaboration. Methods Based on the methodological framework of the scope review,we systematically searched domestic and foreign databases including the Cochrane Library,CINAHL,Embase,PubMed,Web of Science,CNKI,VIP,Wanfang and SinoMed. The time limit for database retrieval was from the establishment of each database to March 2023. The selected studies were summarized and analyzed. Results Totally 13 articles published between 1991 and 2022 were included. The obstacle factors that lead to ethical conflicts in nurse engagement of interprofessional collaboration can be divided into 3 categories, including personal factors(insufficient professional knowledge, differences in professional values),interprofessional factors(imbalanced power between professions, lack of respect and collaboration between cooperators) and environmental factors(insufficient resources and staffing,high-intensity workload, and high-pressure work environment). At the same time, in view of these 3 aspects, it puts forward some countermeasures, such as enhancing professional knowledge, strengthening interprofessional education, improving understanding and cooperation among medical workers in different wards, providing medical practitioners with a team-based,ethical work environment that facilitates interprofessional collaborations and open dialogues. Conclusion It is of great significance for nurses to get involved in interprofessional collaborations,in order to ensure high-quality nursing services. Further studies are needed for the development of education and the core competency framework construction for interprofessional collaborations in nursing. More attention should be paid to develop the organizational atmosphere at the same time,so as to continuously improve nurse’ capabilities in interprofessional collaborations.

Objective To describe the change trajectory of cardiac rehabilitation information needs and self-management in patients undergoing percutaneous coronary intervention (PCI) at different time points, and to explore the correlation between them. Methods A longitudinal study was conducted. From June 2021 to January 2022,186 PCI patients who met the criteria were enrolled in the cardiology ward and cardiovascular clinic of a tertiary A hospital in Jiangxi Province. General information questionnaire, the Cardiac Rehabilitation Information Needs Scale and Coronary Heart Disease Self-Management Behavior Rating Scale were employed for follow-up at 1 day before surgery,3 days after surgery, and 1,3,and 6 months after discharge to evaluate the patients’ cardiac rehabilitation information needs and self-management behavior. Results 165 cases were finally included. The total demand for cardiac rehabilitation information and the scores of each dimension in perioperative PCI patients showed an overall upward trend, but the demand for diagnosis and treatment reached the peak before surgery and within 1 month after discharge, showing a dynamic change overall. Within 6 months after discharge, the total demand for cardiac rehabilitation information,self-management and scores of each dimension of PCI patients showed an overall downward trend, but the demand for risk factor management showed an upward trend within 3 months after discharge. Within 6 months after discharge,self-management,daily life management and medical disease management of PCI patients showed a downward trend, while emotional cognitive management showed an upward trend. There was a positive correlation between them at 1 month after discharge(r=0.185,P<0.05),and a negative correlation between them at 6 months after discharge(r=-0.174,P<0.05). Conclusion The information needs,self-management and dimensions of cardiac rehabilitation in patients with PCI change dynamically during perioperative period and within 6 months after discharge. The self-management of PCI patients was positively correlated with the total demand for cardiac rehabilitation information at 1 month after discharge, and negatively correlated at 6 months after discharge. It is suggested that medical staff should strengthen the follow-up management of patients and meet the needs of patients in all aspects of disease rehabilitation, which can further improve the self-management ability of patients and help to reduce the rate of cardiovascular events and readmission.

Abstract445)   HTML0)    PDF (1068KB)(5)       Save

Objective To evaluate the effect of threshold-pressure inspiratory muscle training on pulmonary rehabilitation and quality of life in asthma patients by meta-analysis. Methods We searched the Cochrane Library,PubMed,Embase,Web of Science,Wanfang,CNKI,VIP,and CBM databases for randomized controlled trial studies on the effects of inspiratory muscle training on pulmonary rehabilitation and quality of life in patients with asthma. The search time is from the establishment of databases to January 6,2021. After the quality evaluation,meta-analysis was performed using RevMan 5.3 software. Results A total of 10 publications including 456 patients were included, and the meta-analysis showed that threshold-pressure inspiratory muscle training improved patients’ forced vital capacity[SMD=0.90,95%CI(0.68,1.11),P<0.001],forced expiratory volume in a second[SMD=0.62,95%CI(0.41,0.83),P<0.001],peak expiratory flow [WMD=0.32,95%CI(0.01,0.62),P=0.040],maximum voluntary ventilation[WMD=11.89,95%CI(6.42,17.37),P<0.001],and maximum inspiratory pressure[SMD=29.76,95%CI(18.15,41.36),P<0.001],reduced their dyspnea[SMD=-0.51,95%CI(-0.95,-0.06),P<0.001],and increased their 6-min walk distance[WMD=77.13,95%CI(24.58,129.67),P=0.004]. However, the difference in improving their quality of life was not statistically significant[SMD=1.21,95%CI(-2.11,4.52),P=0.480]. Conclusion Threshold-pressure inspiratory muscle training can significantly improve patients’ lung function indicators, enhance respiratory muscle strength, reduce dyspnea, and increase 6-min walking distance in asthmatic patients, but the effect in improving quality of life is not significant, and more clinical high-quality studies need to further verify its effectiveness.
